Fingerprint information for all EDU internship/field experience students!

On April 29, 2008, the governor signed into law House Bill #2566. This new law affects all EDU: interns/field experience! It is now a law that you must possess a valid IVP fingerprint clearance card in order to be placed in a school for an internship/field experience.

It is imperative that all students complete the process for their fingerprint clearance immediately. This process can take 6-10 weeks. Any delay in the fingerprint clearance process will affect a student’s progress in the EDU: internship/field experience.

 

The card must be in the student’s possession at all times.

 

If your current DPS Fingerprint Clearance Card does not have an IVP number on the front, you will be asked to complete a new IVP fingerprint clearance process prior to certification with the Arizona Department of Education. Some districts may also require you to possess an IVP card. The most efficient and secure process is detailed below:

 

Visit one of the following Community Wellness & Safety of Arizona locations. They have the IVP Fingerprint Clearance/Teacher Certification Application packets.

  • Community Wellness & Safety of Arizona – Gilbert Location 522 N. Gilbert Rd., Suite 104 (2nd floor) (on Gilbert Road and Juniper, South of Guadalupe Road) 480-892-4295 Hours: Monday – Friday, 8:00 AM – 5:00 PM
  • Community Wellness & Safety – Tempe Location 8400 S. Kyrene Road, Suite 122 (1st floor) (on Kyrene Road, North of Warner Road) 480-222-5722 Hours: Monday, Wednesday, Friday, 8:00 AM – 5:00 PM (closed Tuesday/Thursday)
  • Community Wellness & Safety - Phoenix, Scottsdale, Peoria, Glendale 4730 E. Indian School Rd. Suite 124 Phoenix, AZ 85018

Take the following:

  • A valid Driver’s License, state or government issued ID card or passport
  • A cashier’s check or money order for $69 payable to the Arizona Department of Public Safety (DPS). No cash or personal checks accepted!
  • $15 in cash or a valid credit/debit card for the Community Wellness processing and print costs
