Complete a Non-Credit Registration Form
This may be obtained from the Student Union or you may complete the registration online through My.Maricopa.edu. For information on online registration and payment, please contact the Fitness and Wellness Department.
Complete an Smart Start Packet (Which includes YMCA Waiver, EMCC Waiver and PARQ)
You can download these forms using the links to the right.
You can also get copies in the Student Union.
Print sign & date them then bring them to the Student Union.
Anyone under the age of 18 will need a parent or guardian to also sign the forms.
