General Admission Requirements
- High School diploma or GED
- Pass HESI A2 Placement Test
- Complete admission packet (includes health declaration, required immunizations and lab results, requirements for obtaining a level one finger print clearance card and a separate criminal background check, CPR certification for health care providers, options for funding your education, and more)
Applicants seeking admission to the Practical Nurse Program must see an advisor. The advisor will assist you in completing the application process.
FOLLOW THESE STEPS:
STEP 1: SET UP YOUR ONLINE STUDENT CENTER
This can be done online anytime from anywhere! Go to my.maricopa.edu and click on New Student? Start Here. (Already a Maricopa County Community College District student, click on Student Center.) Follow instructions provided to obtain your Maricopa Enterprise ID (MEID) and password. (New students who do not wish to use their social security number must complete a Student Information Form through Enrollment Services in person or via phone at 623-935-8888 to receive a confirmation number for online set up).
Locate Student ID Number
This number is different from your MEID.
- In the Student Center, under Personal Information, click on Demographic Data.
- Make a note of the Student ID number which will be needed for all campus services.
Activate Your Official Maricopa Student Email Account
In your Student Center, click on the Official Student E-mail button.
- Enter your MEID.
- Enter your Password.
- Click Login.
All official college communications will be sent to your official student e-mail.
STEP 2: TAKE THE PLACEMENT TEST
Successfully complete the HESI A2 (Admission Assessment)
The HESI A2 is a test specifically designed for entrance into a Nursing Program. It will evaluate your Reading Comprehension and Math skills and compare that to the level needed to be successful in a Nursing Program. The HESI A2 can be taken at various Maricopa Community College campuses, and the cost is $37.
Preparation for the test can be done through various study guides available in college bookstores and libraries.
Passing Score: English Composite = 85% or higher, Math = 75% or higher
STEP 3: MEET WITH SOUTHWEST SKILL CENTER ADVISOR
The SWSC Advisor (CNA and PN) will help you complete your admission packet. If you would like to get a head start on your admission requirements, you can download a packet now. Inside the packet, review the Steps for Admission. You can meet with an advisor by appointment or on a walk-in basis. Your advisor will guide you on when and how to complete the forms and answer any questions you may have about your SouthWest Skill Center program.
