ADD/ADHD: School records such as school psychologist report or other special education records. A letter from a licensed psychiatrist or medical doctor is best.
Psychiatric Illness: Medical records or letter from your psychiatrist. A clear diagnosis and doctors names, credentials and signature must be on the document.
Physical Disabilities: Medical records or letter from a medical doctor. A clear diagnosis and doctor’s name, credentials and signature must be provided on the document.
Please remember we cannot determine eligibility for services and accommodations without a clear diagnosis and the name, credentials and signature of the licensed professional making the diagnosis. All medical records are kept strictly confidential.
