Veterans Services Forms
Request for VA Certification:
This form needs to be completed and turned into the Veterans Services Office after you enroll EVERY SEMESTER.
VA Standards:
This form needs to be completed only once upon your initial certification with the Veterans Services office.
Change of Place or Training:
This form needs to be completed if you are changing PARENT schools for VA Benefits, or if you are changing your declared PROGRAM of STUDY with the veterans office.
