Admissions and Records Forms

EMCC offers the following e-forms for your convenience, which can be submitted online using your MEID and password:

• General Upload Form (enrollment verifications, SWSC transcript, admission letter)
• Identification Authentication/Declaration of Citizenship or Status
• Legal Name/Legal Sex Update Form
• Program Plan Update Form
• Refund Appeal
• Special Admissions 
• Student Information Update Request (address, phone number, email, SSN)
• Under 18 Financial Responsibility and Consent

All PDF forms require a signature and can be submitted using the General Form Upload Link.
