Electronic Forms

For your convenience, Maricopa Community Colleges has created electronic forms for students and Maricopa employees to electronically complete and submit through a secured portal using their MEID and password. Upon completing the required form(s), you will receive an automated message stating the form has submitted successfully and whether further action is required from you. These electronic forms are reviewed by EMCC Student Business Services Associates within 1 business day.


  • Employee Tuition Waiver (for eligible Maricopa Community College employees): For additional information related to eligibility, guidelines and terms of use please review the MCCCD Tuition Waivers and Handbook. Your waiver must be received no later than 4:00PM on your tuition due date.
     
  • FERPA Release and Tuition Assistance Form: In compliance with the federal Family Educational Rights and Privacy Act of 1974 (FERPA), a college/university is prohibited from providing certain information from your student records to a third party, such as information on grades, billing, tuition and fee assessments, financial aid (including scholarships, grants, work – study, or loan amounts), and other student record information. This restriction applies to all third parties, including, but not limited to: your parents, spouse, or sponsor. Some exceptions to the disclosure prohibition exist. Please attach your Tuition Assistance form here as well.
     
  • Stop Payment Form: By completing a Stop Payment Form, you are authorizing Maricopa County Community College District to void and/or place a stop payment on the MSRP refund check that was issued to you. Requests for stop payments will be processed after a mailed check has been missing for fourteen (14) business days from the date of issuance.
     
  • Student Debt Authorization: The Student Debt form authorizes EMCC Student Business Services to pay a debt at EMCC for a past term from the current or anticipated Title IV Federal Student Aid. Any remaining funds will be disbursed to you through the Maricopa Student Refund Process (MSRP). By completing this form, you understand that you will not be eligible for a book advance due to the past due debt. (Total prior award year charges, including tuition/fees and other charges cannot exceed $200).
  • Student ID Photo Upload: Estrella Mountain Community College will issue a EMCC student ID to students who are currently enrolled in credit hour classes. The first student ID will be free of charge. Your student ID will be mailed to you.
